I respectfully request admission to the MOC as a Member and agree, if admitted, to abide by the MOC’s Charter, By-Laws, and Rules. I am a US citizen or legally in the US, over 21 years of age, in good standing in the community, and a legal resident of Miami Dade, Broward or Monroe County, Florida.  To review my qualifications as a prospective member, I authorize the MOC to conduct a Background Check, and Credit Check on my person, as deemed necessary, for $100 plus tax which shall be non-refundable and which shall be charged in addition to my MOC Initiation Fees ( where applicable), payable at the time of my presentation of this application for membership to the MOC. If I am not approved my Initiation Fee will be returned within 60 days from non-approval. Upon admission to membership, I agree to pay all monthly charges to my account, including dues and all future assessments adopted or assessed by the MOC when due. Also, as required.
